When you visit a new doctor for the first
time, the receptionist will ask you to
fill out a new patient form to begin
collecting your medical history.
Likewise, when we receive your prescription,
we will call you to gather your patient
history as it pertains to vision care.
Even if you have already purchased vision
correction products at Visions of Canada,
we will want to update your history.
To help us to help you even better, we have a
client preparation form
form that will give us all
the important information we need to make your
new thin lenses for you properly. |

You can print on-line the client preparation form to fax back to us, or scan it
and attach it to your email to us. Once you've sent us the form, give us
a call on one of our toll free numbers. |
If you are a new Visions of Canada client, expect to spend about 10 minutes
with us on the phone. If you are already on our records, we will do an
update and this will take less time. |
If it is inconvenient for you when we call, you can call us back when you
have put aside a block of time. Remember to use our toll free numbers
when you call to avoid charges. |
One of our qualified opticians will ask you questions about your current eyeglasses
and/or contact lenses, your current health status, and your history of vision correction.
We need a thorough understanding of all the likes and dislikes about your previous
eyeglasses in order to determine how much improvement in your
vision you can expect from your new precision perfect thin eyeglasses. |
